Fee For Service is available for those who do not have these plans with sliding scales if appropriate

It is important that you provide us with accurate and complete insurance information. Contemporary Care will submit bills to your insurance company and will do everything possible to advance your claim. However, it may become necessary for you to contact your insurance company or supply additional information to expedite payment.

What is a co-payment?

A co-payment is a set payment made at the time a service is received. Co-payments can vary greatly among insurance plans, so check with your provider to find out how much you may have to pay. We ask that you pay your co-payment on the date of service.

What is a deductible?

A deductible is the amount of money you must pay before the insurance company begins covering part or all of your medical expenses. Deductibles can vary greatly among insurance plans and types of service. Please check with your insurance company to find out if you have a deductible.

Do we accept fee for service?

If we do not take your insurance, we do accept fee for service patients and charge highly reasonable self-pay rates with sliding scales for those who cannot afford our rates. We will provide you documentation and help you get paid from your insurance as an out of network benefit.

What if you do not take my insurance for TMS or ketamine therapy?

You can still do TMS and Spravato/ketamine with us using ‘out of network’ benefits that we will negotiate on your behalf prior to embarking on the treatment.
